MILITARY CASE MANAGER

REPORTS TO: CLINICAL DIRECTOR OF MILITARY PROGRAM and Director of Business Development

STATUS: Non-EXEMPT 

 

Aurora San Diego is seeking to hire a qualified individual to provide direct and indirect patient care and case management services to active duty military patients receiving inpatient care for substance abuse, mood disorders and/or Posttraumatic Stress Disorder.

 

Essential Duties:

  • Provides ongoing case management to active duty patients
  • Attends treatment team meetings weekly
  • Assists in the development and implementation of treatment plans
  • Communicates with referral sources and other appropriate parties regarding patients’ progress
  • Conducts family therapy sessions when indicated
  • Facilitates patient groups as scheduled
  • Coordinates and implements patients’ discharge
  • Attends and participates in department and other program, or clinical, and administrative meetings
  • Supports maintenance and development of clinical program or services
  • Maintains effective communication with all necessary departments

 

 

Education:  Must have one of the requirements below

  • Registered Nurse with previous military medical/mental health case management experience
  • Master’s Degree in Social Work
  • Master’s Degree in Marriage and Family Therapy

 

Licensure:

  • Clinical license/certification to provide clinical services in California OR license eligible in California

 

 

Experience/Knowledge:

  • A minimum of one year post-graduate work experience or

  • Experience working with military mental health providers and case managers

  • Understanding of military culture and protocol

 

Desired Qualifications:

  • Professional experience working with military families or personal military lifestyle experience
  • Knowledge of the difficulties of dealing with the requirements of military life and familiarity with Posttraumatic Stress Disorder
  • Knowledge of military and community resources appropriate for patients
  • Strong oral and written communication skills
  • Ability to work effectively both independently and as part of an interdisciplinary treatment team
  • Describes effectively Aurora’s Military Treatment Program to Military Base personnel
